The concern was misdiagnosed by a different electrician and they confirmed everything is fine and requested this permit to be Voided.

Right side main disconnect outside is broken and no longer able to disconnect voltage to the right breaker panel inside. This is a dangerous condition as electrical servicing and emergency disconnection by first responders is no longer possible without replacing the disconnect. We recommend replacing both disconnects since it is likely the other can fail in the same way will prevent another shutdown replacement event.
Replace main breaker in outside electrical panel. Install correct grounding.
